Care plans provide direction for personalised care for a client / patient. A care plan is organised by the individuals' specific needs and incorporates the continuity of their care. In its simplest form, the care plan is a means of communicating and organising the actions related to an individuals' care.
A care plan should be individual to the patient. Ideally where possible they should have as much involvement in the development of the plan as they reasonably can. Care and support should help the patient be able to:
1) live independently
2) retain as much control over their life as possible
3) have the best quality of life possible
4) keep as much dignity and respect as possible
5) participate in society on an equal level with others
A care plan (also known as a care and support plan, or support plan if you are the carer) sets out how care and support needs will be met. It must set out the following:
1) any needs identified by a period of assessment
2) whether the needs meet the eligibility criteria (including the extent if met)
3) the desired outcomes of the suggested care and support (in conjunction with the patient)
4) the personal budget associated with the care
5) information and advice regarding what can be done to reduce the needs in question, and to prevent any delay to the development of such needs in future
6) the needs that the authority is going to meet - and how this will be achieved
7) support or requirements that the carer may need in achieving the above aims

Booking
Please call us to get a quote. Once your booking is confirmed, we will then send you your joining instructions! No downloads, no hassle. Just create a login and input the code we send you to start the course.

Assessment
After you have completed your assessment (including multiple choice questionnaire), your digital certificate is sent to the email address used to login as soon as the course is completed.
If you have not received your certificate, please first check your spam box. If it isn't there, please call and we will get this resent.
Do courses expire?
There is no expiry date on course credits. Once a credit has been bought it can be allocated at any time. However, should the credit have been allocated to the delegate, each learner then has 12 months to complete the course.
In other words, unallocated course credits have no expiry date. If for example you ordered 1000 licences, and allocated 600 courses in year 1, you would have 400 licences left to allocate in year 2.
